Let’s be completely honest—the vanilla romance and family systems in The Sims 4 are far too easy and predictable. Sims fall in love in a day, pregnancies are always perfectly planned, and everyone lives happily ever after. Where is the drama? Where is the realism?
Sims 4 WooHoo Wellness (RPO) Mod: The Ultimate Pregnancy & Relationship Overhaul
If you want your gameplay to feel like an actual life simulator with complex emotions, real-life consequences, and deep family dynamics, you need the WooHoo Wellness & Pregnancy Overhaul mod.
Note: The brilliant creator, Lumpinou, has officially renamed this massive project to the Relationship & Pregnancy Overhaul (RPO), but most of the community still affectionately calls it WooHoo Wellness. Let’s dive into why this is a permanent staple in our mod folders.
What Does the WooHoo Wellness (RPO) Mod Actually Do?
This isn’t just one simple mod; it is a massive collection of up to 19 different modules. It completely rewrites how Sims handle romance, WooHoo, fertility, and family planning.
Here are some of the most game-changing features it brings to our saves:
- Pregnancy Preferences: Sims finally have their own opinions! They can actively want children, be entirely against having them, or be unsure. If an anti-family Sim accidentally gets pregnant, their reaction will be incredibly realistic (and dramatic).
- Fertility & Protection: No more guaranteed pregnancies. Sims have varying fertility levels, and they now have to actively choose to use protection during WooHoo (or risk the consequences).
- Teen Pregnancies & Family Reactions: It introduces a fully fleshed-out teen pregnancy system with unique moodlets, family drama, and difficult choices.
- Paternity Testing & Cheating: If a Sim gets pregnant while having an affair, you can demand paternity tests. Spouses will actually react to infidelity based on the severity of the cheating (flirting vs. WooHoo).
- Custody & Separation: Sims don’t just magically break up and forget each other. They can request temporary separations, fight over child custody, or arrange co-parenting agreements.
- Diseases (WTDs): Sims can now contract and spread WooHoo Transmitted Diseases, requiring them to visit a doctor and have awkward conversations with their partners.
How to Install Lumpinou’s RPO Mod
Because this mod is massive and modular, the installation is slightly different from a standard piece of custom content.
- Download the Collection: Head over to Lumpinou’s Patreon or website and download the RPO
.zipfile. - Keep the Core Files (Crucial!): The mod relies on “Core Library” files to function. You absolutely must keep the required Core files in your Mods folder.
- Pick Your Modules: The beauty of RPO is that you don’t have to install everything. Don’t want STDs or Teen Pregnancy in your game? Just delete those specific module files! Keep only the
.packagefiles for the features you want. - Move to Mods: Place your chosen modules and the required Core files directly into your
Documents > Electronic Arts > The Sims 4 > Modsfolder. - Enable Settings: Ensure “Enable Custom Content and Mods” and “Script Mods Allowed” are checked in your game options.
Frequently Asked Questions
Here are the quickest answers to the most common questions players search for when setting up WooHoo Wellness/RPO:
Does this mod conflict with WickedWhims or WonderfulWhims?
No, they actually work perfectly together! Lumpinou specifically designed RPO to be compatible with WickedWhims. If you use both, the game will seamlessly merge WW’s menstrual cycles and birth control with RPO’s relationship and preference systems.
Are my Sims going to be sick or infertile all the time?
Not unless you want them to be! RPO comes with extensive in-game settings. You can click on any Sim, open the “Relationship & Pregnancy Overhaul” pie menu, and manually cheat their fertility levels, cure diseases, or change their pregnancy preferences instantly.
Is the dating app feature included?
Yes! Module 16 includes the “Meet&Mingle” dating app, allowing Sims to create profiles, set relationship goals, and go on blind dates right from their phones.

– WTD module: UTIs left untreated cause hospitalization, ut this feature doesn’t work properly as of now. If your Sim gets to that point, cure the disease with the cheat under “Actions” then “WTD Cheats”.
– Teen module: in some people’s games, teen get no pregnant belly. Everything else works as intended.
– Abortion: for now I removed ‘ask to accompany to abortion’ as it refuses to work properly now and the sims don’t go properly. It’ll come back whenever I’ve found a workaround.
– Pregnancy Acceptance: the ‘accepted pregnancy’ trimester moodlets may vanish upon traveling. It is advised to not travel if the moodlet disappearing would bother you. Not sure I can solution that one anytime soon.
– Alternative Pregnancy:
– the ‘surrogate pregnancy’ trimester moodlets may vanish upon traveling. It is advised to not travel if the moodlet disappearing would bother you. Not sure I can solution that one anytime soon.
– DON’T ACCOMPANY YOUR SURROGATE SIM TO THE HOSPITAL!!!! This will cause the option to send away the kids to not show. There may be other circumstances causing the interactions to give the surrogate babies to their parents to not show up on the baby. For these cases, there are debug interactions in place.
